How to get a job while living in another state?

I live in Nashville and I am trying to get a job in Chicago. I have sent out numerous resumes, with only one decent response back. What I am trying to figure out is should I have a Chicago address and phone number on my resume, instead of Nashville. I think this is hindering me in my job search. I have over 10 years of collections with hospitals and with Ford Motor Credit.
